I {name-1}, willingly adopting the pet from his/her foster parent / Adoption Coordinator.

I agree to take full responsibility for the care of this pet. I agree to abide by the adoption conditions outlined below and I am committed to the care of this pet throughout the lifetime of the pet and will not abandon this pet for any reason.

As the Adopter, I agree to the following terms and conditions of adoption: 

1. I have adopted the pet to care for her/him throughout her/his lifetime and I am doing this after consulting my family member(s) who wholeheartedly agree to this adoption
2. I agree to allow the foster parent or a representative of the foster parent to visit my home or premises as and when they want, to visit for this pet, to ensure the terms of this agreement are being kept
3. I am aware that if there is any violations of this agreement then under the Prevention of Cruelty to Animals Act, 1960 legal action can be initiated against me by the foster parent or any NGO or animal welfare officer
4. I agree to keep this pet in my home/owned premises and provide healthy food and fresh water for the pet at all times
5. I will not chain this pet for long periods of time (over 2 hours) except while taking it out for a walk on a leash (If required)
6. I will obey all applicable laws governing the control and custody of animals and will register this pet with the local municipal corporation as per the rules
7. I will provide all medical care and treatment needed by the pet including but not limited to yearly vaccinations, checkups, etc.
8. I am adopting this pet for myself and not as a gift for anyone else
9. I will not sell or give away this pet for any reason
10. I will ensure this pet is never left unattended with children below the age of 8 years
11. I will ensure this pet will be neutered/spayed between the age of 6 months for a kitten and 8 months for a pup, irrespective of whether I adopt a female or a male animal
12. If the pet gets pregnant then I as the pet owner will take responsibility to get the babies adopted to suitable families and get this pet spayed/neutered at the earliest possible time based on a veterinary doctor’s advice
13. I clearly understand that abandoning a pet under any circumstances will attract legal action against me under Prevention of Cruelty to Animals, Act 1960.
14. If for any reason in the future the ownership of this pet needs to be changed, I will do that it in consultation with the foster parent/rescuer and/or local animal welfare organizations and follow the proper adoption procedures as indicated by them
15. I will deworm this pet as per the veterinary doctor’s advise
16. I will get this pet vaccinated yearly for anti-rabies and other vaccines as suggested by the veterinary doctor I have read all the terms and conditions of adoption form and understand and agree with all the terms and conditions for adoption of this pet.
